Job Summary: Program Counselor is responsible Provides comprehensive case management services to youth in care and underprivileged youth ages 14-21 in a designated county. The tasks are, but not limited to conducting intakes & assessments, school/home visits, and referrals to appropriate services. This includes facilitating program sessions and independent living skills workshops, service learning projects and conducting community outreach. Position may require some weekends and travel.
Duties and Responsibilities:

Manage an assigned case load of (1:20) of youth in the LEVEL UP Program

Orient all clients to the LEVEL UP program

Use the completed assessment document and develop in collaboration with the youth an individualized case plan and update quarterly

Must have a written plan for each youth

Conduct job placement research and referrals

Document any and all communication with caregivers

Must document all case activity in the agency case management system

Must maintain all signed case plan agreements in the case files

Must maintain contact with the youth throughout the job placement

Attend all staff or management meetings are required

Provide any necessary data to management as required

Maintain strict confidentiality of the youth information unless it is a specific service provider that is directly related to the youth’s individualized case plan and in accordance with agency policy
Minimum Requirements

Bachelor’s Degree in Human Services or related fields

Minimum of two years’ experience in working with children

Excellent verbal and writing communication skills
-
Excellent organizational and computer skills
-
Successful completion of state required background checks, central registry, sex offender registry, FBI finger printing, and SLED
