Being a Change AgentProgram Counselor provides comprehensive case management services to youth in care and underprivileged youth ages 14-21 in a designated county. The tasks are, but not limited to conducting intakes & assessments, school/home visits, and referrals to appropriate services. This includes facilitating program sessions and independent living skills workshops, service learning projects and conducting community outreach. Position may require some weekends and travel.
Manage case load of (1:20) of youth; Must have a written plan for each youth; Conduct job placement; Must document all case activity;
Must maintain all signed case plan agreements; Attend all staff meetings; Maintain strict confidentiality of the youth information.
